Two Maternity Wards
Ignác Fülöp Semmelweis was born on 1 July 1818 in Buda, then part of the Austrian Empire and now part of Budapest, Hungary. He studied medicine in Vienna and in 1846 began working at the Vienna General Hospital’s maternity clinic.
The hospital had two maternity wards. In the first clinic, women in childbirth were attended by doctors and medical students. In the second, they were attended by midwives. Semmelweis noticed something deeply troubling: the death rate from puerperal fever, also known as childbed fever, was far higher in the doctors’ ward than in the midwives’ ward.
Childbed fever was a devastating infection that struck women shortly after giving birth. In some years, more than one in ten women in the doctors’ ward died from it. The midwives’ ward had a much lower death rate, and no one could explain why.
The Discovery
Semmelweis investigated every possible difference between the two wards. A crucial difference was that doctors and medical students performed autopsies as part of their training, while the midwives did not.
After a colleague died from an infection contracted during an autopsy, Semmelweis noticed that the symptoms resembled those of childbed fever. He concluded that doctors and students were carrying something from the autopsy room to the maternity ward on their hands.
Semmelweis did not know what that something was. He had no microscope evidence of bacteria, and germ theory had not yet been developed. But he reasoned that if doctors washed their hands in a disinfecting solution, whatever they were carrying could be removed.
In 1847, he introduced a policy requiring doctors and medical students to wash their hands in a solution of chlorinated lime before examining patients. The results were dramatic: deaths from childbed fever in the doctors’ ward plummeted to levels comparable with the midwives’ ward.
Rejection and Frustration
Despite the clear evidence, Semmelweis’s handwashing policy was not widely adopted. Many doctors were offended by the suggestion that they were causing their patients’ deaths. The idea that invisible particles on their hands could transmit disease seemed implausible to many in an era before germ theory.
Semmelweis’s appointment in Vienna ended in March 1849. After remaining in Vienna for a time, he returned to Pest in October 1850, where he continued to promote hand disinfection. He grew increasingly frustrated and angry at the medical establishment’s refusal to accept his findings. His writings became more heated and confrontational, accusing doctors who ignored handwashing of being murderers.
By 1865 Semmelweis’s health had seriously deteriorated. He was admitted to a psychiatric institution in Döbling near Vienna on 31 July and died there on 13 August, aged 47. Historical accounts have debated the precise circumstances surrounding his final illness and death.
A Pioneer of Infection Control
Semmelweis did not discover germs, and he did not establish germ theory. The modern understanding of microbial infection developed later through the work of multiple scientists, including Louis Pasteur, Joseph Lister, Robert Koch and others.
What Semmelweis did was something different but equally important. He used careful clinical observation and mortality data to identify that contaminated hands were transmitting disease, and he demonstrated that hand disinfection could prevent those deaths. His method was empirical: he observed a problem, tested a solution, and measured the results.
Today, Semmelweis is recognised as a pioneer of infection control and hand hygiene. Hand hygiene is now a cornerstone of infection prevention in healthcare worldwide, and Semmelweis’s work in Vienna remains one of the clearest early demonstrations that disinfecting hands could dramatically reduce maternal deaths.
